Bet its an edit error since the accession - someone has probably searched for "The Duchess of Cornwall" and replaced it with "The Queen" without factoring in the HRH needing to be changed to HM.
I proof read so many student reports where people do similar, use a search and replace facility on word or the like but forget pronouns, subject names etc.
