Beauteous as it is, I can only imagine the headache from the weight. In the old days of tiara hair there were braids and hairpieces that made it a bit more tolerable. Queen Maxima has the same issue many women of her age have and her hair is not as abundant as it once was. Though I bet she could have the court jeweler refit  some padding and a more evenly distributed weight at the t he bottom of the tiara to make it tolerable

The Kochert Tiara was designed by the House of Kochert in 1901. It was made as a gift from Emperor Francis Joseph I to his grandniece, Archduchess Maria Anna of Austria.

It seemed to me to be one, like the Strathmore Rose, that could be worn low, across the forehead, if desired.
